The novel coronavirus, COVID-19, has been declared a worldwide pandemic by the World Health Organization. COVID-19 is extremely contagious and is believed to spread mainly from person-to-person contact. Although COVID-19 causes mild symptoms in some individuals, it can lead to severe illness and even death in others.
The Colburn School has put in place certain cautionary requirements to help reduce the spread of COVID-19, including requiring everyone who enters the campus to complete a health screening app with a result that clears them to come onto campus, and having their temperature checked upon entrance to the campus. In addition, Colburn School requires face coverings, physical distancing, and frequent hand-washing for anyone on campus. Also, participants in certain programs or group activities on campus will be required to take a COVID-19 rapid test and test negative for COVID-19.
Despite these measures, and understanding that Colburn cannot guarantee that I will not become infected with COVID-19 on campus, I hereby agree to comply with these requirements.
Also, I acknowledge that a COVID-19 rapid test does not produce an accurate result 100% of the time and that a negative test result does not guarantee that I do not have COVID-19. Furthermore, I acknowledge the contagious nature of COVID-19, and I voluntarily and knowingly assume the risk that I may be exposed to or infected by COVID-19 by coming onto the Colburn campus.
If I feel sick or exhibit any symptoms (including, but not limited to, fever, cough, or shortness of breath) associated with COVID-19, or if I have been exposed to anyone with COVID-19 or who has such symptoms, I agree that I will not enter the Colburn campus.
Finally, I hereby release, covenant not to sue, discharge, and hold harmless Colburn, and its employees, agents, and trustees, from any claim, action, action, lawsuit, cost, or expense of any kind arising out of or relating to the possibility of COVID-19 infection on campus or to the requirements set forth herein. I understand and agree that this release encompasses any claims based on the actions, omissions, or negligence of Colburn, or of its employees, agents, and trustees, regardless of whether a COVID-19 infection occurs before, during, or after my presence on the Colburn campus or participation in any Colburn program.
